Password Reset Ticket Template

Password reset tickets are common, but they still need care. The goal is to help the user quickly without putting account security at risk.

Template

  • User name: Full name and username or employee ID.
  • System: The account or app that needs reset.
  • Impact: Is the user blocked from working?
  • Identity check: Confirm the approved verification method was completed. Do not write private answers in the ticket.
  • Action taken: Reset password, unlocked account, forced password change, or sent reset link.
  • User confirmed: Yes, no, or waiting on user.

What not to include

Do not put passwords, security answers, one-time codes, or personal identity documents in the ticket notes. If proof is needed, follow the company-approved process and document only that verification was completed.

A password ticket should be short, secure, and easy to audit later.
